Our Community Health Workers support individuals and families experiencing homelessness, living with chronic health conditions, or needing help reducing barriers to school attendance. The Community Health Worker program offers care coordination to help navigate resources, access community services, and address immediate needs. Through referrals from Great Rivers HUB and local school districts, our CHW team advocates for the whole family to help them accomplish their goals.

How we do it:
The Y Community Health Worker program was enhanced with support on Y Giving Day, and as a result, our team is better able to meet the immediate needs of their clients. One example is that during the holidays, our CHWs were able to provide gifts for the children in their caseloads.
